Forms
Define the information users need to fill in when they place an order. You use a form template by binding it to a product, which you select in Products (商品管理). Fields can add a surcharge, and a field can also let users choose a player.
Form list
The list shows the ID, template name, description, reference count, status, creation time and actions.
- References (引用数): the number of products bound to the template. Click the number to see the products that reference it.
- Enable / Disable (启用 / 禁用): disabled templates do not appear in the Form Template (表单模板) dropdown of the Product Editor (商品编辑); products already bound to the template are not affected.
- Delete (删除): only templates that no product references can be deleted, and deletion cannot be undone.
Create / edit a form
Click Create Form (创建表单), or Edit (编辑) in the list, to open the Form Editor (表单编辑) page:
- At the top, enter the Template Name (模板名称), which is required, and the Template Description (模板说明), which is optional.
- On the left, click Add Field (添加字段) to add form fields one at a time and set their properties.
- The phone preview on the right shows the order page in real time.
- Click Save (保存). If you click Back (返回) while there are unsaved changes, you are asked to confirm.
You can drag field cards to reorder them, and you can also copy or delete them. Fields without a field name are not saved.
Field types

Field settings
- General Settings (通用设置): Field Name (字段名称), the title users see, such as Game ID; Field Type (字段类型); Placeholder (占位提示); and Required (是否必填).
- Surcharge Settings (加价设置), for Switch and Number Input: a switch adds the surcharge once when it is turned on; a number input is charged as the entered value × the surcharge amount.
- Options (选项管理), for Dropdown, Multi-Select Dropdown, Radio Buttons and Checkboxes: each option has an Option Name (选项名称), a Value (值), which is optional and defaults to the name, and an optional Surcharge (加价).
- Tag Filter (标签筛选), for Select Player: limits the players users can choose by player tag; the default is All Players (No Tag Limit) (全部打手（不限标签）). The player the user selects becomes the designated player for the order.
- Image Settings (图片设置), for Image Upload: the Max Images (最多上传张数), from 1 to 9.
- Value Range (数值范围), for Number Input: Min (最小值) and Max (最大值).
- Field Dependency (字段依赖): shows a field only when another field meets a condition. Trigger conditions include Show When Filled (有值时显示), Show When Empty (为空时显示), Equals (等于指定值), Does Not Equal (不等于指定值), In List (在列表中) and Not In List (不在列表中). When a condition needs values, separate multiple values with half-width commas (,).
Note
Changes to a form template immediately affect all products that reference it. Orders already placed keep the content filled in at the time of ordering, which you can view under Form Information (表单信息) in the order details.
